Has anyone else had this happen, and if so what did you do on the resubmit?
GF Submitted a touchstone that had a template (Intro to sociology, touchstone 1). She downloaded the template, and filled in the blanks. Sophia flagged it as plagiarism detected. I ran it through Grammarly twice; once as is, and once with just her writing. None detected on her writing, but plagiarism detected on the filled in template. So, it appears that it’s detecting the template itself. Makes sense. Stuff I turned into turnitin at excelsior saw the copied and pasted questions and things like that. But, the question is how do you get Sophia to not flag their own template that they want filled in? Or, maybe we are both reading it wrong and she is supposed to answer the questions without using the template? Any experience/thoughts on this?

Did software just give you a result that a percentage is similar, or did a person grading it take off points for it? If it just came back that way from Turnitin or similar software, it's probably no big deal. I turned in something just last week. It said 28% similar. But all of it was within quotes, references, headings, etc., so I didn't worry about it, and it wasn't a problem. That 28% was like 1% of this paper, 4% of another, and so on.

Software rejected it, and said it wouldn’t be graded, and to resubmit.

Remove the template and create your own headings.

If you remove the template, you might get dinged on grading for not using the provided template. I would strongly recommend contacting Sophia support before doing that.

She emailed them. They looked at it, found no plagiarism in her writing, and graded it. Makes me wonder how often this happens, and why they don’t have it set to not flag their template?

Yeah, you don't want to deviate from what they require, do your thing and email support to get it corrected. Nothing more is required...
