Nevermind, I asked ChatGPT.

I just helped my daughter complete her application.  The date of birth format we used was MM/DD/YYYY.

We also had a problem with the application accepting her cell number.  W tried multiple number cell numbers in my family and finally skipped the cell number as it is not required to apply.
