Straighterline Bio Lab Invoice

Had previously enrolled into SL Bio lab course years ago and purchased a lab kit second hand which was perfectly fine back then.  Took an extended break and finally returning to complete my degree and it turns out now they require you submit a picture of your esciencelab box with the access code and a invoice/order confirmation. 

My box has an access code, but we do not have an invoice.  If anyone has a PDF copy of their invoice, or even a picture of one so I can recreate it, it would be appreciated.
