I'm missing a step 4: View InstantCert and if you want, email them as well with your request.
Since that's the way to cancel the membership for PayPal users, they should be able to assist in setup of the account as well.
Note: Isn't a Mastercard Maestro a Debit Card and not a full usage MasterCard? I guess that's why it failed.
