Do I need textbooks? Do I need to pay for extra stuff besides the exam?

Do you already have a subscription to Straighterline? It's $59, etextbook and everything required is included at that price.
If you already have a membership subscription with SL, I would highly recommend taking more courses to offset the membership price.
Just make sure you're able to complete the courses before the end of membership. Also, use as many coupons as possible.

SL membership, SL course fee. Use a coupon.
No exam fee, except if you want the rush exam (about $5). Course fee includes digital textbook.

What if I do not pass the exam? Do I retake with extra fee?

I do not have subscription. All I need is Intro to Sociology because I already have all electives and general courses.

With SL, you'd have to retake the course for the entire course fee.

If you just need this one class, then SL is not your best option unless you get some good coupons. You can take the CLEP exam for $80 + testing fee, or the TECEP for $117.
