What does a transfer credit look like on a Charter Oak credit registry transcript?

Hey folks,
I was wondering what a credit registry transcript looks like from Charter Oak? My wife needs to have 30 colleges credits for the NYC Department of Education to get her salary bump. But, due to new NYC Dept. of Ed requirements, FEMA credits accepted must have been completed by Jan 2016 (which she did) and must be on a transcript no later then spring 2016. Now I know i can just do it with Frederick and save the headache but it would be nice to save the 1000 bucks and transfer it to Charter Oak. So was curious to know if anyone had a sample from Charter Oak with FEMA on it.

My FEMA credits to Charteroak only says:

FEM 109 Animals in Disaster P
