Hello...I am a disabled man who is looking for an institution in NYC(preferably Brooklyn) that delivers DSST exams AND provides an application to take the tests. I need to provide written documentation of my disability in order to receive the accommodations that I need.

Welcome to the forums!! I don't actually live in NY but if you go to this website you can search for DSST college testing centers in NY and specifically Brooklyn. You can specify whether you want colleges that administer DSST's or accept DSST's. I don't know whether they have any application, but you can call them if someone else here doesn't post anything. The phone numbers are listed.

I don't understand what you mean by provides application, but this lace is pretty good. You can take test any day. Many schools that host the tests only hold exams once in a while.
