I signed up for the medical terminology course as recommended on the wiki. I actually like this subject and feel confident to take the test. However I'm a little confused where I do that - the instructions here [1] say to do it through ProctorU, but when I go to my courses [2] and view the course, there is a link to a site called Questionmark that has a "start test" option. So I'm confused which to do.

It's both. ProctorU is does the proctoring, questionmark is the testing software that TESU licenses. You make your appointment with ProctorU and start the session with them. Once the proctor authenticates you, they have you open up the exam. The proctor will then enter their access code.
