I took it 2 weeks ago and there was not 1 formula question on the test. Bossman is right, as long as you did 100% on quizzes you should be fine.

Well, they didn't allow a "formula sheet" as one of the allowed items for the proctored final either (or at least not when I took it a couple months back), so maybe this has changed recently? That being said, I did have 1 or two formula questions. Pretty sure Future Value and Simple Interest came up for me.

I never used the scratch paper or calculator for that exam. However, I always had the test code to unlock the exam written on a piece of paper and I would hold it up to the camera and show the front and back of it to show I didn't write anything on it. I also showed everything I could in the room scan - even the floor all around the computer area. I figured, better safe than sorry. I never had a single issue. So I guess you can have those things and just show them on the recording and explain it if you want.
