Lastly (I think), should the letter be sent through the Postal Service, e-mail, other?? Who do we send it to or contact?

I think that's certainly an allowable thing to do. As long as you're "enrolled," you are their customer able to do everything from courses to graduation.
I'm 19 and had no trouble getting a waiver. Admissions wanted a letter detailing why I would be a good candidate for TESC (it ended up being one page for me) with attached, scanned copies of all my unofficial transcripts (considered "unofficial" because they were not sent directly to the college from the test site). A reply email allowing me to apply came shortly thereafter (within a day or two).
The only thing that is really going to help with the waiver is the amount of credits--I had 72 at the time which is plenty to validate an ability to participate in college-level work. Waiting nine months isn't really going to change much--either way she's under 21. I'd say don't wait! Jump on it. Because once you start dealing with TESC, you have to slow down a bit to their schedule (no longer running your show by yourself). I wish I would have applied earlier!
So if you want to shoot an email explaining your situation, they'll help you. Or if you want to go ahead and explain the situation--AND attach a letter/scanned copies of all her transcripts (I only attached college scores, no highschool), then I'd say go for it. You have nothing to lose and I would feel comfortable saying you're certain to get in. Sent mine to

They never actually sent me a "waiver," they just replied saying: "This is sufficient, thank you! Please fill out our online application." That's it. So I applied, paid DSST and CLEP $20 to send in my transcripts, meanwhile got an acceptance letter via email. I'm now waiting on an "evaluation" to figure out where all the credits fit.
Biggest thing for you guys is to check graduation dates if she has any courses to finish. Good luck! This is all from my personal experience, lemme know if you have other questions!
