Being accepted and enrolled are two different things.
To clarify, being enrolled means you have registered for a course at TESU and that course has been completed or begun at least 10 days ago. This can be a regular course or a TECEP exam. If that didn't happen, you are not enrolled.
Also, getting your TESU account flagged for SDC doesn't require a specific form. But there are two ways to do it: The first way requires you to have specified "Study.com affiliate" on your TESU application. The second option requires contacting TESU after the fact to have the flag added after you've had your transcripts evaluated.
In either case, to qualify (or prove eligibility) you need to complete at least one ACE or NCCRS credit-eligible course from Study.com, send your Study.com transcript to TESU and have the transcript evaluated, and wait for those courses placed on your academic record.
If you applied as an SDC affiliate, you should be good at that point, though you should confirm. If you did not do have that checked on your application you need to contact TESU via phone or open a ticket to ask them to add the Study.com corporate affiliation to your account.
To confirm your affiliate status you can click on your Profile link in OSS and look under the Student Status field. If it has been applied, it should show "CPPEN Corporate Choice" followed by "Enrolled" or "Applied" in that field (this also shows whether you are an actively enrolled student or only an applicant). If it doesn't include the Corporate Choice text at all, you are NOT flagged for the SDC discount and will need to contact TESU again.
