Welcome to the board, it's great you're asking questions to see if there are any programs that fit your needs, but without knowing more info from you and what is requested from the addendum and template, it's hard for people to see the full picture of what you need and what you bring to the table. There are various ways of getting that done, competency based degrees such as the UMPI MAOL might be your best bet.
